Endometriosis is a condition, in which tissue similar to the endometrium, grows and develops outside of the uterine lining. If untreated, this disease leads to other conditions (adhesions, obstruction of the fallopian tubes), resulting in severe cases of female infertility. Annually the gynecology department of the clinic of Friedrichshafen successfully treats about 3 000 patients with endometriosis. The department is equipped with modern sonographic, laparoscopic and laser devices, which helps to perform minimally-invasive interventions on pelvic organs in a safe and effective way. Our patients report about excellent long lasting results without pain and relapses.
Approximately 30% to 50% of women with endometriosis are infertile. Women suffering from pelvic pain, irregular menses and infertility should consult a specialist and get through a diagnostic study as soon as possible. Inflammation due to endometriosis releases substances which inhibit normal egg function. Besides endometriosis may lead to adhesions in the tubes, which leads to tubal blockage. Formation of scars and adhesions not only block the fallopian tubes, but also interferes with implantation of the fertilized egg.
If the ovaries are affected with endometriosis, cysts may develop, which leads to anovulation. As a result, women, suffering from endometriosis have difficulty getting pregnancy and need professional medical treatment.
Only the experienced gynecologist can iIdentify endometriosis. After listening to the complaints of the patient, the specialist will hold a full clinical and gynecological examination to rule out other women's diseases. In most cases additional research is required to diagnose endometriosis:
Currently there are two main methods of treatment of the endometriosis: conservative and surgical treatment. A drug-hormonal treatment helps to reduce pain, decrease the endometrial implants or make them disappear. Hormone replacement therapy is often done in combination with surgery. It should be noted that hormone therapy is not indicated in patients who want to get pregnant.
Conservative treatment includes:
If the conservative treatment is inefficient, the surgical treatment is indicated. Surgical treatment aims at the removal of endometriosis, whereby the herds are often treated with a laser beam, or get coagulated. Especially effective is the so called combined therapy - surgery, combined with conservative (medication) treatment. The choice of surgical approach (laparoscopic or open surgery) and medication depends on the scale of the endometriosis process.
Laparoscopic treatment of endometriosis: Endometriosis surgery is done under general anesthesia. During the surgery an optical instrument (laparoscope) with a small video camera is inserted through a small incision. Required instruments are introduced through further small cuts in the abdomen. The surgeon sees the operating area in a monitor and can carry out the necessary measures. In diagnostic laparoscopy, the surgeon assesses, which organs are affected with endometriosis. Afterwards the endometriosis lesions are removed or obliterated with heat, electricity or laser beams.
At the end of the surgerical treatment, a drainage is sometimes placed in the abdominal cavity, so that woundafluid can be derived. The patient shoud stay at hospital for another 3 days. The cost of such treatment makes up 7 000 – 10 000 euro (with the view to the scale of endometriosis development)
Laser treatment of endometriosis: The efficiency of laser treatment makes up 95 percent. The laser beam easily penetrates the skin, leaving no scars and adhesions on the internal organs. Under the influence of laser power adhesions are heated to a high temperature and evaporate from the surface of the affected organs.
Endometrial cryotherapy: this method of endometriosis treatment involves freezing temperatures, which help to scrap the uterus lining, affected with endometriosis. The procedure is performed under local anaesthesia and lasts about an hour. The freezing process is monitored by ultrasound, which helps to destroy the target area, without damaging the healthy tissues.
According to statistics, chances to get pregnant rise by 70% after surgical treatment of endometriosis. Moreover the IVF treatment is twice as efficient in patients without endometriosis, which implies that the conservative and surgical treatment should be performed prior to applying for methods of assisted reproduction.
