Chronic cardiac insufficiency treatment

The specialists at the cardiology department at Friedrichshafen Clinic (Germany) achieved good results in treatment of different cardiac diseases, including cardiac insufficiency – inability of heart to perform its main pump function. This disorder is a leader among the heart diseases, diagnosed in patients who are older than 35 years old.

The head of the Department MD Detlev Jäger is an experienced specialist with broad scope of cardiological qualifications. More than 1500 patients with cardiac insufficiency are successfully treated at this department annually.

The following factors can cause cardiac insufficiency

Due to various reasons, the cardiac muscle weakens and pumps less amount of blood into the veins than your organism needs. All organs suffer in this case; they do not get enough oxygen:

  • cardiac muscle affection (myocarditis, myocardial infarction, cardiosclerosis, rheumatism, systemic lupus erythematosus);
  • heart overload due to the blood flow difficulties (arterial hypertension, aortic stenosis (narrowing), pulmonary artery stenosis, valvular insufficiency);
  • rhythm disturbance;
  • external cardiac compression (pericarditis, tamponade);
  • abnormality of the cardiac muscle flexibility (amyloidosis, fibroelastosis).

The symptoms of the cardiac insufficiency: dyspnea, dry cough and edemas. Intensity of these symptoms tells us about the particular stage of the disease. In the early stage, dyspnea comes during physical activity. In case of full-blown cardiac insufficiency, dyspnea is permanent even in the quiescent state. It increases after the slightest physical effort, even during a conversation. In case of severe form, dyspnea comes suddenly and has a form of a suffocation attack. Edemas often appear on the legs in the evening and pass by the morning. Edemas become permanent along with the disease progression. Muscle weakness during cardiac insufficiency is caused by both the reduced blood flow in the muscles and dyspnea that comes after any kind of physical load.

Accurate diagnosis of cardiac insufficiency and its degree determines the treatment specific character. The applied examination methods in the German clinics allow to make a prognosis of disease and prescribe an appropriate treatment course, including surgical operation if needed. For example, if heart diseases or great vessels stenosis are detected, surgery has to be performed as soon as possible not waiting for a severe form of the disease.

Diagnostics by cardiac insufficiency

The patient examination algorithm in the treatment process, that was developed by the German specialists, is very clear:

  • laboratory blood analysis;
  • lungs radiography (congestion of blood in the lungs, heart sizes);
  • electrocardiogram (ECG) at rest and with a load, daily monitoring;
  • EchoCG at rest and with a load (heart wall thickness and its chambers sizes are determined as well as the volume of pumped blood);
  • electron-emitting and multispiral CT (to detect sclerosis and coronary vessels calcinosis);
  • coronarography (determination of sizes and localization of the affected areas in myocardium).

The additional diagnosis methods are perfusion scintigraphy, magnetic resonance arteriography with contrasting substances application.

Cardiac insufficiency treatment

Usually, they start the cardiac insufficiency treatment with medicamental therapy. Doctors prescribe diuretics for edemas elimination and heart relief, cardiac glycosides to improve cardiac muscle functioning, beta-blockers for rhythm normalization, nitrates for vessels dilation.

In case of evident rhythm disorders, which are especially dangerous by onset of the ciliary arrhythmia paroxysms, medicamental therapy is supplemented by realignmental therapy – artificial pacemaker (cardiostimulator or cardioverter-defibrillator) implantation. Pacemaker implantation, performed at German clinics can be a key to healthy life.

The main target of cardiac insufficiency treatment is:

  • Elimination of symptoms - shortness of breath, fatigue etc.
  • Protection of target organs (heart, kidney, brain, blood vessels, muscles)
  • Improvement of the "quality of life"
  • Reducing the number of hospitalizations
  • Improved prognosis (life extension)

In the severe cases the heart transplantation is conducted. This step, however, is rather radical and the specialists of our do apply to it, when all other methods of cardiac insufficiency treatment have been ineffective.
