First steps in infertility treatment

After a detailed examination you decide together with your specialist about the treatment steps. First approaches are:

  • Cycle observation. For example, with cycle monitoring, the observation of the woman's natural menstrual cycle through ultrasound and blood tests, the optimal period for fertilization can be calculated.
  • Hormonal treatments. Often, hormonal imbalances in a partner are responsible for infertility. By taking medication or regulating weight in overweight or underweight people, the hormone balance can normalize and pave the way for a - natural or possibly artificial - fertilization.
  • Artificial fertilization. If cycle observation or hormone treatments have no effect, you can also opt for artificial insemination.

If pregnancy does not occur naturally, there are the possibilities of artificial insemination (so-called assisted reproduction). These are usually preceded by hormone treatments that optimally prepare the body for the various treatment options.

Reproductive medicine is regulated in Germany by the Embryo Protection Act. In addition, the German Medical Association and the Joint Federal Committee of Doctors and Health Insurance Funds have issued guidelines for action. In Germany the regulations are very strict. Prohibited are, for example, the use of foreign ova, surrogacy, embryo cloning, sperm selection (except for serious gender-related hereditary diseases) or the use of semen.

Intrauterine insemination

In semen transfer (intrauterine insemination), the sperm are transferred directly into the uterus - usually when the partner has too few or insufficiently mobile sperm. The semen transfer is considered a less stressful method for women - despite previous hormone treatment.

Before transfer, the semen is processed to increase the chance of fertilization and prevent allergic reactions. The average success rate is around 5 to 10 percent per trial. After several transmissions occurs in about 10 to 30 percent of cases pregnancy.

In vitro fertilization (IVF)

In vitro fertilization (IVF) takes several weeks: After hormone treatment, the woman's eggs are removed from the ovary and mixed in a test tube with the partner's sperm. If fertilization occurs and the fertilized eggs continue to develop, an embryo is returned to the uterus.

An IVF is a complex process, the embryo does not always nest in the uterus. Often, therefore, two to three embryos are transmitted - resulting in 20 percent of IVF treatments to multiple pregnancies. The success rate of an IVF treatment is around 25 to 30 percent, the live birth rate at 15 to 20 percent.

Intracytoplasmic Sperm Injection (ICSI)

Intracytoplasmic Sperm Injection (ICSI) is a special IVF treatment. It is used when there is no fertilization of the egg due to sperm quality during semen transfer or IVF treatment. In ICSI, ova are taken from women following hormonal treatment, and a single sperm is injected directly into the egg. Successful fertilization and cell division, up to three embryos are transferred to the woman's uterus.

TESE and MESA

Testicular Sperm Extraction (TESE) and Microsurgical Epididymal Sperm Aspiration (MESA) are other methods of artificial insemination. It may happen that there are no sperm cells in the man's seminal fluid. In some cases sperm can be obtained directly from the testes (TESE) or the epididymis (MESA) by means of surgery. The subsequent artificial insemination is then carried out according to the ICSI method.

I have done two failed IVF treatments in Stuttgart, and I am looking for other options. Both times, we had two embryos fertilized and transferred but no pregnancy was achieved. I am very interested in testing the embryos before they are transferred. I am also interested to see how your clinic works with patients who do not live in the area. Also, do you accept records from other clinics rather than making clients go through all the tests again? Thank you for any information you can provide.

Hello,
I am doing research on behalf of a friend who’s wife was a cancer patient thankfully she is a survivor, however after treatment they have been advised that the chances of her been able to carry a baby is than 4% due to the damage cased to her eggs during the cancer treatment. So my question to is can you help? Is there any treatment you can provide to improve the chances? What are the costs?
